HOW MUCH STAFF TIME IS REQUIRED?

• Bar code version: 1 - 3 minutes/patient.
• KIOSK: Just add paper. KIOSK will read instructions and age/gender specific questionnaires to patients, in English and Spanish.

COMPARISONS TO NATIONAL DATA NORMS?

You can translate scores obtained with the Dartmouth COOP Clinical Improvement System to the MOS/SF 36. Our relationship with Mercer enables other comparisons.

THE COST?

The price of the standard bar code version is $250 ($350 for dialysis) plus $15 for shipping and handling. If needed, a bar code scanning wand costs $125. The standard system has a 45-day money back guarantee. The KIOSK version costs $7,500 and includes all hardware, an AT&T warranty and maintenance agreement, and the three versions for adolescents, adult and geriatric patients.
